40 Year Old Male with Bi-Polar Affective Disorder
Including Initial Adult Psychiatric consultation, baseline blood test review, collaboration with GP ,medical review, historical document and referral letter review, Initial Psychiatric Assessment Document (IPAD) and prescription.
6 months of weekly Psychotherapy.
